Support Within Housing - Mental Health

Program Description

A case management program to support clients to stabilize into their housing, community, and access supports surrounding mental health, addictions, social networks, managing tenancy, and money management.

Access Information

Referrals are accepted from service providers. To make a referral, contact us. A screener will be emailed to the service provider.

Inclusion Criteria:
1. Have an official mental health diagnosis provided by a medical practitioner or clinical psychologist. Or the client has undiagnosed severe persistent mental health and refuses to engage with a mental health professional.
2. Has an experience of chronic homelessness- client has been continuously homeless over the past 6 months or has had multiple shelter stays over the last three years.
3. Have either been recently housed or has a lease signed.
4. The client must be 18 years of age or older.
