Assertive Community Treatment Teams (ACTT)

Program Description

This program offers intensive service during periods of crisis and acute illness to less intense service during periods of stability. For individuals with an Intellectual Disability (DSOER eligible) who are experiencing symptoms of mental illness and who may experience significant behavioural challenges, extended hospital stays and frequent use of emergency services, and/or involvement in the criminal justice system.

Access Information

Referrals accepted from General Practitioners (GP) or Nurse Practitioners (NP) or from community partners with consent from a GP or NP. Documentation supporting criteria for admission required.
Referrals for Leeds-Grenville are coordinated through Leeds-Grenville Rehabilitation Counselling Service.
Referrals for Lanark County are coordinated through Lanark Community Mental Health.
