Program Description

Community Outreach and Support Team (COAST) provides intensive case management and transitional case management services for individuals experiencing serious and persistent mental illness.

The function of COAST is to assist people in moving towards recovery and to facilitate a higher level of independence in the community. The involvement of family, friends and other supports is encouraged and welcomed.

COAST is a mobile service that provides comprehensive and individualized treatment, housing, medication, and rehabilitation (i.e., employment, finances, cooking). The intensity and duration of service is based on each person's needs and wishes.

Access Information

Individuals can be referred to COAST through the intake service.

Specific ICM admission criteria includes: Consumers must have diagnosis that meets the definition of serious mental illness (Axis 1) requirements of disability, duration and diagnosis. They need ongoing long-term support from service providers and require assistance to meet their goals in more than one life domain.

Individuals must meet two or more of the following criteria:
- socially or geographically isolated
- experiencing barriers because of race, culture or ethnicity
- have multiple and complex needs who are at risk for repeated or prolonged institutionalization in health care or correctional facilities
- have had three or more psychiatric admissions within the last two years or have been detained in an inpatient facility for 60 days or more within this period
- have been subject to two or more police complaints/interventions in the last 12 months or have been incarcerated in a correctional facility for 30 days or more within this period
- have been recently evicted from housing, or homeless or living in shelters
- have current problems with drugs and or alcohol
- consistently use crisis services in the community
- require frequent contact with their support systems.
