Assertive Community Treatment Teams (ACTT)

Program Description

Supports offered include: working with the client to find housing based on personal preferences; support in accessing basic necessities such as personal identification, food, clothing, health and income support; comprehensive bio-psychosocial assessment; collaborative development and implementation of an individualized recovery plan; outreach supports and the development of a trusting relationship between the client and the service provider team; assistance to achieve/maintain optimum health and wellness; and linkage to community resources (social, recreational and/or primary health services).

Access Information

Referrals are accepted from anyone, including self-referrals.
Referrals are through The Access Point. Please call or fill out the application form online.
Once the application is received, a service navigator will call the individual to set up an appointment to do an intake assessment.

Eligibility criteria includes serious mental health challenge and homelessness, or at-risk of homelessness.
