Program Description

The Transition Home provides safe and affordable housing with flexible supports in a homelike atmosphere. The home is staffed 24 hours a day.

The program is for youth and includes the following services:
- Subsidized supported housing
- Individualized case planning
- Individual and group counselling
- Life and social skills training
- Assistance in preparing the young person to make the transition back home or to a more independent living situation

Access Information

Referrals from all sources including self. Call for services. Admission to the Transition Home is through an assessment.
