Program Description

The Community Mental Health Promotion Program (CMHPP) Assertive Street Outreach Team provides case management and harm reduction services for individuals living with mental illness, and who are homeless. Services include harm reduction supplies, harm reduction education, referrals and information about community resources, and access to primary care.
