Medical Inpatient Withdrawal Management
The elective inpatient medical withdrawal service has a multi-disciplinary team of addiction counsellors, social workers, nurses, nurse practitioners and psychiatrists. The team provides a comprehensive medical assessment to determine the appropriateness for admission and to develop a care plan that will guide an individual through the withdrawal process as safely as possible.
Close medical monitoring is provided for the duration of the admission.
Upon discharge, the individual can choose to continue to be supported through our outpatient community programming.
It is important to note that the program is not an emergency/crisis service and that access to the program must occur through the following process:
1. A referral form must be completed by a community health care provider. This can be located on the Humber River Health website-search under programs-Mental Health and addictions.
2. Once the referral is received, a staff member will contact the individual in order to book an initial screening, time, which will be conducted by a designated program nurse to understand the concerns of the patient.
3. If appropriate, a medical assessment will be booked with WDM to determine admission suitability and to formulate initial treatment plan. The individualized treatment plan is focused on pharmacological interventions specific to the substance(s) of concerns as follows:
- Acute Withdrawal Management medications
- Urge reduction medications
- Opioid Replacement Therapy (i.e. Methadone, Suboxone, Sublocade)
- Psychiatric treatment recommendations for any underlying concurrent mental health concerns
