Support Within Housing - Mental Health
A transitional support housing program for men 55 years of age and older with mental health issues, substance use issues, street involvement, homelessness, and unstable housing.
On-site supports and services include: case management; support around harm reduction and substance use; supportive counselling; support with activities of daily living; linkage to nursing; life skills development; social and recreational activities; volunteer opportunities; peer programs; support transitioning to permanent housing; follow-up supports.
Referrals come from hospitals, City of Toronto, the social service organizations.

FSTH Staff team is available 8 am to 12 midnight Monday to Sunday. Staff office is closed on statutory holidays. Security on site from 12 midnight to 8 am.
