Assertive Community Treatment Teams (ACTT)
Assertive Community Treatment Team (ACTT) is a multidisciplinary team of mental health professionals who provide client-centred, highly individualized treatment, support and rehabilitation to people with severe and persistent mental illness.
The program helps clients achieve their goals, improve their health and quality of life, and live as independently as possible.
Self-initiated referrals are accepted.
The program accepts people who: are diagnosed with severe and persistent mental illness, particularly those with a diagnosis of schizophrenia; are unable to function independently in the community due to their mental illness in the areas of self-care, productivity and leisure; have a high number of admissions to hospital; require intensive service to maintain treatment, rehabilitation, and support in the community.
