Assertive Community Treatment Teams (ACTT)
Assertive Community Treatment (ACT) is a community-based model of care for individuals who have severe and persistent mental illness. The program reaches out to people assertively where and when they need help directly in their home and in the community. ACT brings a multidisciplinary team of healthcare professionals together to provide highly integrated and individualized services for their clients.
An individual can discuss eligibility with current treatment team and/or physician. Generally referrals come from outpatient/inpatient attending psychiatrists or from the hospital. ACT teams can have waiting lists of varying lengths depending on the availability of ACT services as well as the appropriateness and priority of the referral.
