Program Description

PAARC Transitional Case Management supports clients who have recently been to the hospital or who are at high risk of hospitalization, and require intensive community-based supports around their substance use or a concurrent disorder.

Individuals can self-refer and are encouraged to contact the program directly. Referrals also accepted through one-Link.
