Case Management - Mental Health
Intensive case management provided by staff who visit clients one to two times a week and provide phone support. Staff assist individuals in finding housing, assessments and goal planning, service coordination, mental health monitoring, and activities of daily living skill building.
Referrals are accepted by anyone, including self-referrals.
Referrals are through Access Point. Please call or visit the website.
Eligibility Criteria:
Individuals with a mental health diagnosis and in need of intensive case management services.
