Program Description

Stable housing and intensive case management for people with problematic substance use and/or concurrent disorders who are homeless, at risk of becoming homeless, or are inadequately housed.

Access Information

Referrals are accepted from all sources, including self-referrals.
To make a referral, please call to complete an intake call. From there, Intake will arrange for an eligibility assessment (SPDAT).
