Program Description

Frontenac Assertive Community Treatment Team (ACTT) is a multidisciplinary team which provides outreach, individualized treatment, ongoing and continuing services, linkages, and includes a monitoring and evaluation component. Assertive community treatment is a client-centred, recovery-oriented mental health delivery model that facilitates community living, psychosocial rehabilitation and recovery for persons who have a serious mental illness, have severe symptoms and impairments, and have not benefited from traditional outpatient programs.

Access Information

Referral forms must be completed and are accepted from all sources, including: family physicians, psychiatrists, family members, social workers or the individual requesting service. All referral forms must be signed by the individual who is requesting the service.

Referrals for this program can also be done through AccessMHA's online referral form. For individuals who do not have internet access, call directly to book an appointment.
