Assertive Community Treatment Teams (ACTT)

Program Description

A multidisciplinary team of mental health professionals offering intensive support to individuals with complex mental health and physical health needs. Services include addictions, behaviour therapy, case management, nursing and psychiatry.

Access Information

Referrals are through The Access Point. Referrals can be made by physicians, hospital professionals, mental health and community organizations. Self-referrals and referrals by family members are also accepted.

Eligibility criteria include:
- history of serious mental health problems with impaired functioning in the community; inability to consistently perform activities of daily living
- inability to consistently maintain a safe living situation and multiple recent hospitalizations and/or use of hospital emergency departments.

Also, a minimum of three of the following:
- inability to meet basic survival needs, current homelessness or precarious housing
- criminal justice involvement in the context of mental health and addictions problems in the past year;
- coexisting substance use disorder;
- developmental delay or acquired brain injury;
- acute and/or chronic medical illness which requires intensive support and difficulty effectively utilizing traditional office-based out-patient services.
