Support Within Housing - Mental Health
For adults 59 and over who have a serious mental illness and are homeless or at risk of becoming homeless. Provides assisted living and a more supportive housing environment. Assistance is provided to each tenant, according to their specific needs, in the following areas: grooming, laundry, housing, cleaning, managing finances, taking medication, nutrition and dietary restrictions, social activities, help with personal problems (e.g. counselling) and require personal care support.
Referrals are through The Access Point. Please call or fill out the application form online.
Once the application is received, a service navigator will call the individual to set up an appointment to do an intake assessment.
Client must be able to live independently with minimal support. Very low program turn over. Vacancies occur infrequently.
