Program Description

one-Link is a single point of access for referrals to addiction and mental health service providers within Mississauga and Halton. This includes referrals to Ambulatory Outpatient Mental Health Services at both Halton Healthcare Services and Trillium Health Partners.

Access Information

Primary care, service provider, and self referrals are accepted. Please note that for many services a primary care referral will be required.

Please use the standard one-Link referral form for all adult referrals 16 and over and for outpatient child and adolescent referrals in Halton. For child & adolescent referrals in the Peel Region, please refer to www.wheretostart.ca.
