Intensive/Day Treatment - Mental Health

Program Description

The Wellness Program provides support for individuals identified as frequent users of emergency services, including emergency department, ambulance and police services, homeless or at risk of being homeless, having minor infractions of the law, and those experiencing a mental health or substance use event who don't need admission to a hospital, but need stabilization and care coordination. Staff provides one-on-one sessions with individuals as well as supportive group programs. The program's focus is short-term, timelines are flexible based on client's needs, and collaborates with community organizations.

Access Information

Self-referrals are accepted. Referrals accepted from hospitals and and other community organizations. Call for service. An initial assessment appointment will be scheduled. All referrals are screened for safety and suitability.
