Program Description

The St. Leonard's Reintegration Program provides transitional case management and crisis intervention to individuals who reside in the community and have mental health concerns diagnosed or suspected and involvement in the criminal justice system. The team's objectives are to provide comprehensive case management to address symptom management, medication compliance and assist in managing criminogenic factors in order for successful community reintegration.

Community partner and self referrals are accepted. Service referral/contact requests can be made by calling, and also can be made through the St. Leonard’s Community Services website, identifying the Reintegration Program as the interested point of service.
