Program Description

As an extension of Specialized Geriatric Services in the community, the Intensive Geriatric Service Worker program provides follow-up and intensive service coordination for older adults with complex geriatric issues.

This program works with the broader healthcare system and community to support older adults who may be isolated and have limited resources. Team members work closely with the older adults’ network to promote independence and self-directed care through the coordination of appropriate services and supports.

Access Information

Referrals are received from system-designated sources inclusive of Geriatric Emergency Management nurses, Geriatricians, Geriatric Psychiatrists, Specialized Geriatric Services (SGS) Mental Health Clinicians, Mental Health Inpatient Nurses/Social Workers, Primary Care Outreach Teams.
