Counselling and Treatment - Mental Health

Program Description

For individuals age of 65 and older with a psychiatric concern that is complex in nature or has not responded to treatment, presenting with one or more of the following: cognitive changes, behavioural changes, mood changes, anxiety or psychotic symptoms.

Access Information

Referral is required from a physician or nurse practitioner at the Long Term Care Home.

Documentation required to indicate a complete referral:
- PHQ-9 (if Depression is suspected)
- Updated Medication List

Referrals are through Ontario Shores Central Intake.

Inclusion Criteria:
Memory concerns, responsive behaviours, general mental health (mood anxiety, psychosis). Client must reside in a long-term care home and require geriatric specific care.

Exclusion Criteria:
Developmental Disorder (Dual Diagnosis)
