Program Description

Program provides assistance with opioid adjustments in a chronic pain context, including opioid replacement treatment and linkages to other community-based programs. Chronic pain assessment is conducted by a Nurse Practitioner (NP); including pain history, mental health assessment, medication review, psychosocial history and substance and risk assessment. One on one counselling and virtual chronic pain self management groups are offered as well.

Access Information

Eligibility criteria: individuals experiencing chronic, non-cancer pain, greater than 3 months.
Self-referrals and referrals from another organizations are accepted, but it is preferred to receive referrals from Primary Care.
