Case Management - Mental Health

Program Description

Community Support workers offer long term case management to clients in WoodGreen housing sites or to clients living independently.

Clients living in WoodGreen buildings are required to have an in-person visit with a worker, once a month.
Clients living independently are required to have an in-person visit with a support worker, once every six months.

Access Information

Referrals are accepted from anyone, including self-referrals.
Referrals are through The Access Point. Please call or fill out the application form online.
Once the application is received, a service navigator will call the individual to set up an appointment to do an intake assessment.
Once referred to Woodgreen, the housing manager will arrange an initial interview, followed by a tour or the residence for acceptable applicant.

Eligibility Criteria:
Individuals who have experienced homelessness and are recovering from mental health challenges. Must be able to live independently.
