Program Description

Crisis and short-term support to older adults and their families in the stabilization of responsive behaviours associated with dementia, cognitive impairment, complex mental health, substance use and/or other neurological conditions. Provides referrals and connections to community resources and other required support services.
Services include:
- Risk management and assessments
- Cognitive assessments
- Client and caregiver education
- Referrals to community supports and health programs
- Falls prevention training, assessment and management of environmental and home risk factors
