Support Within Housing - Mental Health
The Supportive Housing Program connects individuals living with mental illness, who are at risk of becoming homeless, to permanent and affordable housing. Intensive case managers support individuals to: explore interests, abilities, skills, strengths and barriers in order to achieve their housing goal; access existing housing and financial resources within the community; access tenant information to inform them of their rights and responsibilities as a tenant; and maintain independent housing.
Referrals can be made by self, family, hospital, physician, psychiatrist, mental health or health professional. Referral forms may be accessed through the website, or call the Information and Referral line. Interim supports offered through various groups and resource centre. Access to this location is by appointment only.
