Support Within Housing - Mental Health

Program Description

The Supportive Housing Program supports individuals who are at risk of becoming homeless. The program helps individuals to obtain suitable housing while receiving support through the Intensive Case Management Services. The program supports individuals to explore interests, abilities, skills, strengths and barriers in order to achieve their housing goal; supports individuals in accessing existing housing and financial resources within the community; assists individuals in accessing tenant information to inform them of their rights and responsibilities as a tenant; and maintain independent housing. GAIN assessment tools are used for referral and screening process.

Access Information

Referrals can be made by self, family, hospital, physician, psychiatrist, mental health or health professional. Referral forms may be accessed through the website, or call for referral. Interim supports offered through various groups and resource centre. Access to this location is by appointment only.
