Case Management - Mental Health

Program Description

The Intensive Case Management provides guidance and support to help individuals develop and maintain essential life skills needed for independent living while improving accessibility to community resources and reducing the incidence of hospital re-admission. The program provides outreach services to individuals who may identify or appear to have a service need; establishes an individualized recovery action plan with a focus on individual strengths and motivations; advocates for the protection of rights and access to a seamless continuum of care; and offers skills development and learning opportunities through psychosocial rehabilitation strategies.

Access Information

Referrals can be made by self, family, hospital, physician, psychiatrist, mental health or health professional. Referral forms may be accessed through the website, or call the Information and Referral line. Interim supports offered through various groups and resource centre. Access to this location is by appointment only.
