Evidence-based psychotherapy through a stepped-care collaborative delivery model for individuals with mild to moderate symptoms of depression or anxiety.

Access Information

Referral accepted by primary care providers or self-referral. Individuals must live in Burlington or have a primary care provider who practices in Burlington.
Primary care providers can refer patients by submitting the centralized referral form to Joseph Brant Hospital Community Mental Health Services.
Individuals may self-refer by calling or completing the online referral form.
