Case Management - Mental Health
Provides a person with a mental health issue, a comprehensive plan of community-based treatment or care with supervision, that is less restrictive than being detained in a psychiatric facility.
Community Treatment Orders are intended for those who have a serious mental illness and a history of repeated hospitalizations. A doctor has examined the individual and believes the person requires continuing treatment and care or supervision while residing in the community.
Referrals are required from physicians or hospital.
Referral is sent or faxed to community treatment order coordinator. Once the referral is received, the CTO Coordinator will perform an assessment to determine the applicant’s eligibility and suitability for our program. If the applicant is not eligible for services, the referral source will be notified.
Inclusion criteria: have major mental illness; have two separate admissions or 30 day stay in psychiatric facility in past three years; or be on previous community treatment order.
