Program Description

The centralized intake team will gather information from you to determine what service best fits your need with local resources.

Access Information

Referrals can be completed by youth, parents and caregivers, and other third parties such as physicians and schools.
Please call or complete the online referral form.

After making a referral, an intake appointment may be scheduled. The intake appointment may take place in the in-person in the office, by video conferencing or by phone.
