WIN (Wellness in Independence) Case Management

Case Management - Mental Health

Program Description

Mental health case management services to individuals requiring follow up from a crisis, or long-term assistance with a goal of reaching an optimum level of functioning in the community.

Services include: supportive counselling; links to resources and support networks; advocacy; ensuring goals are met; medication monitoring and health education; administration of medication within a clinical setting; rehabilitation skills and support; enrichment; encouraging fulfilling and satisfying activities; support with budgeting for food and housing.

Access Information

Referrals can be made by physicians, psychiatrists and health care providers. Referrals can be made via fax.
Clients will be assessed by a nurse after a referral is received to deem appropriateness for this service.

Inclusion criteria:
- willing to work with a nurse from the Victorian Order of Nurses mental health team
- assessed as having a chronic mental disorder requiring ongoing support
- experiencing difficulty with daily functioning and clients referred to the program must have control over their behaviour
- must be able to ensure they will not exhibit violent or aggressive actions towards themselves or others
