Counselling and Treatment - Mental Health
The team supports older adults aged 65+ experiencing mental health concerns, age related emotional changes, major life transitions, or behavioural and psychological symptoms of dementia. Delivered by a team of nurses, social workers, and occupational therapists in collaboration with Psychiatrists from The Royal Ottawa Hospital, this community based service provides therapeutic support tailored to the unique needs of older adults. Through outreach visits, comprehensive assessments, individualized treatment planning, targeted interventions, caregiver education, and system navigation, we help older adults and their families manage mental health challenges, cognitive decline, and dementia related behaviours.
Individuals can self refer through AccessMHA online referral form. For individuals who do not have internet access, or who prefer to call, call directly to book an appointment. Physicians can complete a referral through Oceans, or by fax.
Eligibility:
- must be at least 65 years of age with any identified or suspected acute mental health concern or with neuropsychiatric symptoms of dementia
- admitted to hospital and requires Geriatric Psychiatry consult for dementia with behaviours
- referrals for consult with Geriatric Psychiatrist, with PCP consent only
- reside in Cornwall or the County of Stormont-Dundas-Glengarry or Akwesasne
