Case Management - Substance Use

Program Description

The Rural Wellington Addictions service is delivered in collaboration with the Upper Grand Family Health Team.

The program provides clients with in-person assessments, treatment/care planning and counselling services. Individuals are also supported in accessing other community services based on their identified needs and/or goals. Education and support for friends/family of those who use substances is available. Services are provided by both an addictions counsellor and/or a peer support worker.

This service works together with the Rapid Access Addiction Medication Clinic (RAAM) and Community Withdrawal Support Service (CWSS) to provide addictions support to the residents of Wellington County.
