Outpatient Clinic - Mental Health
Comprehensive Community Support Team (CCST) provides ongoing and episodic care to clients who have reached their potential and require ongoing social, vocational, housing or medication support or who require episodic support to address social determinants of health.
Comprehensive Community Support Team (CCST) is a community-based treatment program for persons with severe and persistent mental illness. The team also supports those whose mental health illness may be influenced by addiction. The approach to treatment is client-centred and is delivered by a team from a variety of disciplines.
Comprehensive Community Support Program - Medication Clinic provides ongoing support to clients who are able to attend independently for appointments to renew prescriptions or for medical monitoring.
All community referrals are physician-based and require that a referral form be completed to determine whether the client meets the admission criteria.
Referrals are processed through a centralized intake process.
