Program Description

Comprehensive Community Support Team (CCST) provides ongoing and episodic care to clients who have reached their potential and require ongoing social, vocational, housing or medication support or who require episodic support to address crises that arise.

CCST - Medication Clinic provides ongoing support to clients who are able to attend independently for appointments to renew prescriptions or for medical monitoring.

Access Information

All community referrals are physician-based and require that a referral form be completed to determine whether the client meets the admission criteria.

Referrals are processed through a centralized intake process.
