Case Management - Substance Use
The Transitional Aged Youth (TAY)/ YOC - Youth Outreach Counsellors program provides specialized treatment and navigational supports to youth who are struggling with substance use.
Working within an inter-professional practice team, the specialized case management service will provide direct care and assist young people and their families to navigate the system of services to meet their individual needs. The target population are young people who may be using poly-substances, including opiates, or are struggling in school, are homeless, or are involved in the criminal justice system and/or living in remote areas.
Referrals may be made by self or other service providers within the youth and adult mental health and addiction systems by contacting the Youth Addictions Intake.
A substance use assessment is not a pre-requisite of accessing services.
