Program Description

The Psychogeriatric Resource Consultant (PRC) provides education, capacity building, assessment, and consultation regarding older adults with increasing complex physical, cognitive and mental health needs associated with difficult to manage responsive behaviours in various community settings.

Referrals to this program are made by self, primary care or community service providers.
Individuals may self-refer by calling the Specialized Geriatric Services (SGS).
Professionals may refer by faxing the referral form. Please call if you require a referral form.
