Program Description

The Dual Diagnosis Case Management program supports individuals, their families and other service providers to develop crisis prevention plans, facilitate community integration, provide short-term service, and coordinate developmental and mental health services. Services are for individuals that have a severe and persistent mental illness causing disability.
