Program Description

This service provides community outreach, assessment, treatment planning, skill development, health teaching, psychiatric consultation, medication monitoring, client and family advocacy, coordination of community resources, public and professional education to individuals who are experiencing severe and persistent mental health problems and require ongoing, consistent support services.
