Program Description

This Supportive Housing Program assists individuals who have substance use issues, who are homeless, at risk of being homeless or are marginally housed, and who want to make positive changes in their lives. The program provides stable housing linked to support services and intensive case management that is flexible to meet individual's needs, goals and service plans.

Access Information

Referrals accepted from all sources. Complete AccessMHA online referral form. For individuals who do not have internet access, or who prefer to call, call directly to book an appointment or come to the office to complete the referral process.
