This program assists in transitioning youth from children and adolescent mental health services to adult addiction and mental health services.

Access Information

Referrals accepted from all sources. Complete the AccessMHA online referral form. For individuals who do not have internet access, or who prefer to call, call directly to book an appointment, or come to the office.
