Case Management - Mental Health
Provides case management services that are available to individuals experiencing a mental illness (schizophrenia, bipolar disorder, clinical depression or other serious mental illness) and/or addictions. The illness must be of a long-term nature and causing an impairment for the individual, i.e., not able to perform daily tasks, keep a job, go to school etc. Services provided by the case management program include: intake and assessment, goal setting, assistance with linkages to other services and resources. The case manager offers regular support to meet the individual's needs. This program also supports individuals in alternative level of care (ALC) beds and prevents homelessness
