Program Description

In-reach services and follow-up care are provided to individuals being released from secured custody. Support and the coordination of mental health follow-up care are provided to integrate individuals back into the community.

Referrals accepted from all sources. Call for referral. Referrals can also be received by contacting the crisis line.
