Case Management - Mental Health

Program Description

Upstream provides a choice of ongoing flexible support services, through mental health case management, and family focused case management. This program provides ongoing psycho-social recreational group activities designed to decrease isolation and expand social networks for all individuals. Emphasis is placed on reducing isolation, education, life skills, and networking.

Access Information

Anyone can refer including self-referrals. Referral is made through Mental Health Community Support Services (MHCSS) central intake located at the Canadian Mental Health Association's (CMHA) Ottawa branch. Contact CMHA Ottawa for referral.
