Program Description

Together with the case manager, the client develops and implements a rehabilitation plan consisting of learning new skills, development supports to achieve their goal of independent living.

Access Information

Written referral form. Client must have diagnosis of major mental illness, and desire to live in North Simcoe. Should need support to live independently and be willing to engage in a rehabilitation process (learn new skills to increase independence).
