Program Description

Together with the case manager, the client develops and implements a rehabilitation plan consisting of learning new skills, development supports to achieve their goal of independent living.

Access Information

Individuals can self refer, be referred by another community agency, family member, hospital, etc. Please complete and fax the referral form, available on the website. Professionals can also refer through Caredove.
Individuals must have a confirmed diagnosis of major mental illness, need support to live independently, and be willing to engage in a rehabilitation process (learn new skills to increase independence).
