Case Management - Mental Health

Program Description

This service is available to seniors with mental health concerns and complex care needs who reside alone or with family and are experiencing difficulty coping in their environment.

Service to Seniors under 65 will be considered on a case to case basis.

Access Information

Priority will be given to Seniors who are at high risk and experiencing increasing frailty with contributing medical and psychosocial stressors.

Risks identified for service:
- caregiver burnout
- home situation is "stressed"
- possibility of abuse has been identified
- change in physical, emotional and/or mental status of either client or caregiver
- social isolation
- access to supports limited or non-existent

Individuals living in a Retirement Home or Assisted Living environment may also be referred.
