Counselling and Treatment - Mental Health
This service can provide psychosocial assessments and risk identification, case coordination as needed, and appropriate therapeutic intervention to seniors with mental health concerns and complex care needs who reside alone or with family and are experiencing difficulty coping in their environment.
Service to seniors under 65 will be considered on a case to case basis.
Referrals accepted from primary care or Ontario Health atHome. Fax completed referral form or refer through Caredove.
Priority will be given to individuals who are at high risk and experiencing increasing frailty with contributing medical and psychosocial stressors.
Risks identified for service:
- caregiver burnout
- home situation is "stressed"
- possibility of abuse has been identified
- change in physical, emotional and/or mental status of either client or caregiver
- social isolation
- access to supports limited or non-existent
