Counselling and Treatment - Mental Health

Program Description

Provides assistance to help transition seniors with complex mental health needs from one environment to another supported environment including Long Term Care Home.

Social work services are also available to families who have a senior with a mental illness living with them, and who are experiencing difficulties.

Access Information

Individuals over 65 years of age must be suffering from a serious mental illness and/or neurodegenerative disease; if under 65 years, must suffer from a neurodegenerative disease process such as Alzheimer's Disease; exhibit behavioural disturbances associated with cognitive impairment interfering with ability to function in their environment; be involved with a healthcare professional.
