Program Description

Provides assistance to help transition seniors with mental health concerns and complex care needs from one environment to another supported environment including Long Term Care Home.

Access Information

Referrals accepted from primary care or Ontario Health atHome. Fax completed referral form or refer through Caredove.

Individuals over 65 years of age must be suffering from a serious mental illness and/or neurodegenerative disease; if under 65 years, must suffer from a neurodegenerative disease process such as Alzheimer's Disease; exhibit behavioural disturbances associated with cognitive impairment interfering with ability to function in their environment; be involved with a healthcare professional.
