Support Within Housing - Mental Health

Program Description

A variety of housing opportunities and support services to assist individuals with a history of serious and persistent mental illness to secure and maintain permanent, safe, affordable, and high standard housing.

Housing support services are designed to assist individuals to live with stability, autonomy, and dignity in the community. The Housing Support Worker will assist clients to maintain stability in the community through short-term intensive services designed to address their housing needs as well as their activities of daily living needs. Individuals will have access to adequate, safe, and affordable accommodations according to their needs and choices. The Housing Support Worker will facilitate the discharge planning process to help individuals with a mental illness who are currently homeless, at risk of becoming homeless or who are being released from a psychiatric facility and require housing.

Access Information

Applicants may refer themselves to the program or be referred by family, friends, mental health or social organizations, by hospitals or other sources.

A waiting list is maintained once the program is full.
