Case Management - Mental Health
Provides individualized support to clients living with a serious mental illness who are at significant risk of involvement or reinvolvement with the criminal justice system.
Supports offered typically include: comprehensive assessment, collaborative development of a personalized safety plan, collaborative development and implementation of an individualized recovery/goal plan and assistance to achieve/maintain optimum health and wellness.
Referrals accepted from police or probation officers.
Inclusion criteria:
- Adults (18+ years of age) who are living with a serious mental illness, or presenting with behaviour suggesting a serious mental health issue with possible co-occurring disabilities including dual diagnosis and other cognitive disabilities. - individual is at moderate or high risk of being charged with a criminal offence or currently facing charges or who had charges or was released from jail within the past year or have five of the following: two or more prior convictions
- current substance use or significant history of substance use
- homeless or at risk of homelessness or three or more address changes in the last year
- current family conflict
- financial stress (applicant's financial situation is a current stressor)
- lacks informal social supports
- subject to two or more police mental health calls or apprehensions within the past year
- history of violence
