Dual Diagnosis Case Management Program

Program Description

Provides individualized support to clients living with a co-occurring developmental disability and a diagnosis of serious mental illness. The team also provides dedicated support to individuals with a dual diagnosis who also have justice system involvement.

The supports offered to clients typically include: comprehensive assessment, collaborative development of a personalized safety plan, collaborative development and implementation of an individualized recovery/goal plan and assistance to achieve/maintain optimum health and wellness.

Access Information

Referrals are accepted from individuals, family members, service providers, health and social service agencies. Please call for information.

For individuals with justice system involvement, referrals must be from designated priority referral sources which include Mental Health Court Support Services, Police, Probation/Parole, the Law and Mental Health program at CAMH, Ontario Shores Centre for Mental Health Sciences' Forensic Unit and other funded Mental Health and Justice services.
