Support Within Housing - Mental Health
Services are provided by psychiatric case managers and community rehabilitation workers to support consumers to live in accommodations of their choice.
The program offers flexible and individualized support which varies in intensity. Assistance in locating and maintaining housing is provided. Rent supplements are available and permanent according to need. The priority of the program is to provide services to people with a serious mental illness who are homeless or at risk of being homeless.
Self, family members, friends, or service providers make referrals.
The referral process includes an intake assessment to determine needs, difficulties, strengths, and goals. Individuals must have a serious mental illness.
